Eagle Creek Family Medicine is currently accepting most insurances. As a courtesy, we will bill any insurance so please check with your insurance carrier to be certain that we are listed as providers on your plan. It is the responsibility of the patient to supply accurate and up-to-date insurance information at the time of the appointment. All new patients are required to have identification, insurance information and payment at the first appointment.
Any co-pay and deductible amounts that are specified by your insurance company must be paid at the time of service. We accept cash, check, Visa, MasterCard and Discover.
